He was predeceased by: his grandparents, Joseph "Jack", Marguerite "Marge" Kepfer and Virginia Mault (Charlie). He is survived by: his parents, Denny Mault (Connie) of Elida and Connie Mault (Kepfer); his daughters, Izzy and Isabel Kathleen Mault; his wife Carla Phalen (Phalen); his sister Tracy Smith of Lima; his nephew Adam Smith of Spencerville; and his father-in-law Michael Phalen (Brenda) of Lima. He is also survived by several nieces, nephews and cousins.

Memorial contributions may be made to a fund that is being established for his daughter Izzy's education.
