He was predeceased by: his parents, Clyde Cook, and Muriel Cook, Sr.; his wife Mary Ann Boerger; his children-in-law, Delphine Cook and Brian'Barney'Montgomery; and his in-laws, Don Boerger (Jean), Jim Boerger, Jackie McKinley, Dorothy Jean' Williams (Ronald), Thomas Boerger, Nick Moore and Ruth Fromm (William).
He is survived by: his children, Mike Cook (April) of Lima, Dennis Cook of Lima, Brian Cook (Stacey) of Cridersville and Teresa Montgomery of Delphos; his sister Patricia Mullenhour (Bob) of Wapak; and his sister-in-law Pat Boerger of Florida. He is also survived by 10 grandchildren; 26 great grandchildren.
