He was predeceased by: his parents, Lloyd Cook and Thelma Cook (Swartz); and his brother Fredrick "Fred" Cook. He is survived by: his wife Linda Cook (Mull); his children, Mark A. Cook (Tammie Joseph) and Tami D. Smith; his grandsons, Troy, Trent and Treg; and his brothers, Lee Cook (Lynell) and Ty Cook (Carole). He is also survived by several loving nieces and nephews.
