He was predeceased by: his partner Tracy Dancer. He is survived by: his parents, Creed Smith and Gloria Smith (Barnett); his uncle James; his aunt Joyce Dean of Kentucky; his friends, Kevin Litsey, Michael Schoonover, Bob Gehr, Mike Whitney, David Whitney and Cam Aller; his son Adam Creed Smith; his partner Loren Bree Howden; his former wife Tracy Mault Smith; and his brothers, Greg Smith (Laura) and Alan Smith (Lisa).
