He was predeceased by: his parents, Andrew Chismark and Helen Chismark (Blackburn); his wife Dorothy L. Chismark (Smith); and his siblings, Annabelle Bozin, Blance Ragozzine, Betty Swank, Jeanne Palipchak, Robert, Charles and George Chismark.

He is survived by: his daughter Nancy Blackburn of Liberty Twp; his grandchildren, Craig Blackburn (Amy) and Courtney Blackburn; and his brother David Chismark of Girard.
