He was predeceased by: his grandson Jared Mazuk; and his parents, Louis Mazuk and Helen Mazuk. He is survived by: his sons, Bradley Mazuk (Gwyn), Gregory Mazuk (Mary) and Jeffrey Mazuk; his grandchildren, Melina, Jaden, Eleanor and Grace; and his brothers, Michael, Gary, Steven and James.

Visitation was held on Thursday, November 14th 2024 from 10:00 AM to 12:00 PM at the Hodapp Funeral Homes - Liberty Township (6410 Cincinnati Dayton Rd, Liberty Township, OH 45044). A funeral service was held on Thursday, November 14th 2024 from 12:00 PM to 1:00 PM at the same location.
