She was predeceased by: her husband Gerald "Jerry"; her parents, T. Leonard Hockett and Maurine Hockett; and her siblings, Ruth Subich (Joe), Charles Hockett (Sara) and Pauline Scherrer (James).
She is survived by: her children, Dawn Tullio (Agostino), Amy Gustaferro (Robert), Paul Burkholder (Julie) and Carl Burkholder (Geraldine). She is also survived by many grandchildren, great grandchildren, great great grandchildren, nieces, nephews, and dear friends.
In lieu of flowers, a donation in honor of Hazel can be made to Methodist Nomads or Lewis Center United Methodist Church.
