He was predeceased by: his parents, Lawrence Hazelton and Esther Hazelton (Morgan); and his siblings, Eileen Schmidt, Marvin, Lawrence Jr. and Charles Hazelton.

He is survived by: his wife Joetta; his sons, James Hazelton (Julie) of Leipsic and Steve Hazelton (Andrea) of Leipsic; and his grandchildren, Jala, Lyndie, Samantha and Aaron Hazelton.
