He is survived by : his wife Donna; his sons, Gary Cook (Cynthia) of Lancaster and Greg Cook (Sally) of Pickerington, Ohio; his grandchildren, Sarah Cook and Alex Cook (Ashley); his stepgrandsons, Jay Darfus and Stuart Darfus; his great grandchild David Thomas; and his nieces, Debbie Henderly and Jana DiDomenico.
