He was predeceased by: his parents, Emil Hornacky and Sibyla Hornacky; and his siblings, Emily, Sibyla Hockman and Jerry Hornacky.
He is survived by: his brother John Hornacky. He is also survived by his children and grandchildren.
In lieu of flowers, the family requests that donations be made in Sam's memory to St. Jude Children's Research Hospital.
