We are sad to announce that on March 20, 2018, at the age of 75, Janet M. Ptak (Lake Milton, Ohio), born in Medina, Ohio passed away. Leave a sympathy message to the family on the memorial page of Janet M. Ptak to pay them a last tribute.
She was predeceased by: her parents, James Smyth and Deloris "Belle" Smyth (Bihn). She is survived by: her husband Gary Franklin Ptak; her daughter Rebecca Berry Gentile of Richfield, OH.; her grandchildren, Mackenzie Nagoda, Jessica Gentile and Nicholas Gentile; and her siblings, James Smyth (Kay), Betty Stiles and Jane Phillips. She is also survived by her parents and her husband.
In lieu of flowers the family requests donation be made to the ALS Society in Janet's name.
