She was predeceased by: her parents, George W. Eddy and Jessie E. Eddy (Lanham); her husband Joseph F. Cook; her son Ralph Cunningham Jr.; and her siblings, Frances Riggs, Daniel W. Eddy, Lois "Fay" Haught and Dale A. Eddy.

She is survived by: her sons, John D. Cook (Cindy) of Norton and Joseph W. Cook of Hartville; and her siblings, Robert "Bob" D. Eddy, Judy L. Little and Thomas L. Eddy. She is also survived by several grandchildren and great grandchildren.
