He is survived by: his mother Dana Hawkins (Lloyd) of Killbuck; his siblings, Sandra Whitley, Tina Galford (Mike) and Arthur Cook (Lori) of Walkerton; and his nieces and nephews, Josh of Indiana, Artie of Indiana, Adam of Indiana, Sherry Cook of Indiana, Jerimie Ellis, Tracy Ellis, Angela Chaney, Ashley Watts, Manda Jackson and Devin Williams.
In lieu of flowers, the family suggests memorial contributions be made to Shriner's Hospital or to the charity of the donor's choice.
