She was predeceased by: her parents, Clonie Columbus and Anna Marie Angle (Moore); her children, Donna Margie Cook, Sandra Marie Cook and David Allen Cook; and her siblings, Nellie Bonner, Herman Angle, Virginia Kay Shamp, Cecil Angle and James Albert Angle.
She is survived by: her former husband Bobby Cook; her children, Arthur Cook (Lori) of Walkerton, IN and Tina Galford (Mike); her husband Lloyd Hawkins; and her grandchildren, Jerimie Ellis, Tracy Ellis, Josh Cook (Billie Jo), Arthur Cook Jr., Adam Cook (Misty), Sherry Cook (Joe), Angela Chaney (Rex), Ashley Watts (Kevin), Manda Jackson (Dan) and Devin Williams. She is also survived by 18 Great-Grandchildren and 3 Great-Great Grandchildren.
