He was predeceased by: his parents, Thomas Warner and Marguerite Slack (Lay); his wife Brenda Park; and his son-in-law Jon Mabrey. He is survived by: his children, Lori Mabrey of Kenton and Rick Slack of Kenton; his brother Jeff Slack (Bobbie) of Forest; his grandchildren, Jessie Lowery (Luke) and Brandon Slack; his great grandchildren, Jason and Liam Lowery; and his brother Body Shop.
