He is survived by: his wife Sherry; his children, Kathy Waites (Gary) of Pickerington, Rob Smith of Ashland, OH and Amy Saffle (Bob) of Mifflin, OH.; his grandchildren, Garrett, Kyler, Kamryn Waites of Pickerington, OH and Hailey Burger of Ashland, OH.; his siblings, Terry Smith (Becky) of San Clemente, CA and Diana Mullen (Daniel) of Springfield, MO.; and his friends, Lyn-Way, Fresenius Dialysis, Dave, Suzie Foster and Phil.
