He was predeceased by: his parents, Bernard E. Lewis and Bertha Griffin (Fultz); and his siblings, Lacey Lewis and Carol Boyd. He is survived by: his wife Mary E. Lewis (Jeffers); his children, Rick Lewis (Marla) of Owingsville and Dixie Black (Tim Martin) of Owingsville; his siblings, James Lewis (Joyce) of Galion, Lester Lewis (Kathy) of Morehead, Mike Lewis (Peggy) of Galion, Stella Barricks (Benny) of Indiana, Doug "Chuck" Lewis (June) of Arizona and Aileen Calton (Roy) of Kentucky; and his grandchildren, Rick Jr. Lewis (Megan), Dalton Lewis, Brittnay Taylor, Terry Taylor, John Cole (Lisa), Kim Cole (Lucas), Alicia Miller (Josh), Abigail Bailey (Dustin), Amy Prince (Chuck), Abby Martin, Corey David (Aubrey), Brock David (Gabby), Dakota David (Amanda), Megan Crissman (Tyler) and Candice Griffith (Christian). He is also survived by son; and numerous nieces and nephews.
