He was predeceased by: his daughter Tiffany Perkins; his parents, Wade Perkins and Genevieve Perkins (Fowler); and his brother Bill Fowler. He is survived by: his children, Alicia Kowalske (Jacob), Wade Perkins III and Chad Perkins (Hanah); his grandchildren, Cody, Dylan, Thomas and Colt Kowalske; his great grandchild Arianna Kowalske; his siblings, Sherry Zinn (Geroge) and Dan Perkins; and his significant other Freckles.
