He is survived by: his daughters, Suzanne M. Perisa of Kingsville, OH, Pamela S. Perisa (Mike) of Hubbard and Jennifer L. Perisa of Hubbard; his siblings, Dennis Perisa (Kathy) of Fowler, Carole Esmond of Cincinnati, Gina Ash (Chuck) of Fowler and Lori Sivey (Lonny) of Columbus; and his grandchildren, Amanda, Justin, Ashlynne, Mia and Sydnee.
