He was predeceased by: his parents, Peter Samulka and Anna Hercha Samulka; and his siblings, Paul Samulka, George Kostecky, Caroline Sue Kostecky, Judy Aggers, Mary Bishop, Helen Clark, Anna Baker, Pauline Samulka and Peggy Dressel.
He is survived by: his wife Shirley Paes Samulka; and his children, John A. Samulka Jr. of Niles, Debra A. Wasserman of New Castle and Susan L. Conroy (Michael) of Hubbard. He is also survived by 5 grandchildren; 4 great grandchildren and numerous nieces and nephews.
