He was predeceased by: his parents, Ralph Cook and Leona Cook (Hagemeyer); his sister Karen Miller; his brother-in-law Robert Miller; and his mother-in-law Susan Evener.
He is survived by: his wife Linda; his daughters, Amber Archambeau (Joe) and Samantha Shaffer (Cliff); his siblings, Sharon Deer (Gary), Thomas Roe Sr. and Sandra Adams; and his father-in-law Thomas Evener Sr.. He is also survived by many nieces and nephews.
