She is survived by: her children, Becky Blade, Daniel Craig and Michael Craig; her grandchildren, Colby Blade and Rachel (Aaron); and her niece Lori Gentry.
A graveside service was held on Friday, April 11th 2025 at 11:00 AM at the Sardinia Cemetery (129 Sardinia-Mowrystown Rd, Sardinia, OH 45171).
In lieu of flowers, the family suggests memorial contributions in Phyllis's name be made out and sent to Ohio's Hospice of Butler and Warren County, 5940 Long Meadow Drive, Middletown, Ohio 45005.
