She was predeceased by: her grandparents, Leslie Mayzlik, Wilma Mayzlik, Virgil Stone and Viola Stone. She is survived by: her parents, Charles Mayzlik of Heath and Jane Mayzlik of Heath. She is also survived by many aunts, uncles and cousins.

She was preceded in death by her paternal grandparents, Leslie and Wilma Mayzlik; maternal grandparents, Virgil and Viola Stone; and beloved cat, "Jake" In lieu of flowers, memorial contributions may be made in Lesley's name to Newark Baptist Temple, 81 Licking View Drive, Heath, Ohio 43056.
