He is survived by: his parents, Herbert Kozak and Margaret Kozak (Tanoury); his wife Susan; his children, Jennifer Liu (Peter Liu), Christina Kozak, Lindsay Englefield (Will) and Michael Kozak (Jessie); his grandchildren, Andrew, Jonathan Liu, Brody Burns, William, Emma Englefield and John W. Kozak; his siblings, Herb Kozak (Susan), Margo Moyer (Bill), Steve Kozak, Chris Kozak (Carol), Lisa Kohn (Dave) and Tricia Kozak; his mother-in-law Rita Eppley; his brothers-in-law, Dave and Dan Eppley (Anne); and his father-in-law Roger Eppley. He is also survived by his children and was very involved in all the activities of his grandchildren.
