Koslovsky; her children, Curt Koslovsky (Lisa) of Cincinnati, Craig Koslovsky (Corie) of Tipp City, Jennifer van Driest (Paul) of St. Louis and Kimberly Plew (Jimmy) of Delaware, Ohio.; and her grandchildren, Emily, Anna, Nicholas, Katherine, Mason, Cameron, Jocelyn, Natalie and Marley.
In lieu of flowers, memorial donations may be made to the American Heart Association.
