He is survived by: his wife Melissa Martin Hornsby; his children, Sarah Cox of Kalamazoo, MI, Kara Hornsby of Hamilton, OH and Dustin Hornsby; his grandson Ethan Cannon of Marion, IN; his brother Brian of Florida; his in-laws, Doug and Nancy Beckman of Ross, OH; and his close friend Belle.
