He is survived by: his wife Ava Burkhart (Taylor); his children, Jessica Kolb (Brian), Jeremiah Burkhart (Sara) and Kimberly Frantz of Greenville Ohio; his grandchildren, Dylan Kolb (Brooke), Brandon Kolb, Madison Kolb, Breanna Burkhart, Kadilynn Tinkham, Adalee Burkhart, Ramee Burkhart, Hailey Frantz, Haiden Frantz and Holton Burkhart; his great grandchild Oaklynn Kolb; and his siblings, Marty Burkhart of Evarts Kentucky and Ester Burkhart of Evarts Kentucky.
