He is survived by: his wife Sandra L.; his children, Jessica Kostalnick (Michael) of Elyria, Megan Davis (Craig Danielson) of North Ridgeville and Matthew Davis (Jennifer) of Avon Lake; his grandchildren, Michael Kostalnick, Emily Kostalnick, Evan Kostalnick and Caralyn Davis; and his brother Richard Davis of Elyria.
