He is survived by: his sons, Samuel, Jonathan, Matthew and James; his wife Kristyn; and his parents, Connie Concoby of Painesville and John J. Prochaska Sr. of Harrison Township, Mich.. He is also survived by numerous aunts, uncles, cousins and friends.
