He was predeceased by: his parents, Shirley Number (Marsh) and Charles Collins Jr. Number; his siblings, Patti Wilmoth, Kim Seals and Gary Collins; and his nephew Jeremy Collins.
He is survived by: his wife Sherri Collins (Seals); his grandchildren, Sydney, Stella, Wallen and T'mir; his parents-in-law, John and Karen Seals; and his siblings, Sherri Boggs (Tim), Chuck Collins (Laura), Lynne Boylan (Bill), Mitch Collins (Krisann), Matt Collins (Lori), Tina and John Seals. He is also survived by his wife and children.
The family requests donations in Tom's memory be made to the Garrettsville-Hiram Rotary Club P.O. Box 184 Garrettsville, Ohio 44231 to continue a part of Tom's legacy.
