He was predeceased by: his parents, Arthur Jr. and Betty Smith Jr.; and his son Jeremiah Lee Smith. He is survived by: his wife Ann; his daughter Heather Marie Smith Queler (Andrew); his siblings, Dale E. Smith and Pamela Smith; his grandchildren, Kylee Marie, Braylee Sophia and Brayden Jeremiah; his brother-in-law Jack Songailo; and also, Gary Harris and Freda Harris. He is also survived by numerous relatives and friends.

In lieu of flowers memorial contributions may be made to Lewy Body Dementia Association, Inc. 912 Killian Hill Road S.W., Lilburn, GA 30047.
