He was predeceased by: his father Robert. He is survived by: his mother Maryann; his wife Kelly; his children, Shayne (Carrie) and Aaron; and his siblings, Bobby Applegarth, Cliff Applegarth, Gayle Potts (Joel), Wendy Pipo (Tom), Lori Lofton (Ed) and Robyn Thomas.

In lieu of flowers, friends may make a memorial contribution to Donate Life Ohio, Second Chance Trust Fund, Ohio Department of Health, 246 N. High St., Columbus, OH 43215.
