She was predeceased by: her sister Shelva Perkins; and her grandparents, Roger, Carla Clark and Beatrice Fountain. She is survived by: her parents, Lavonda Fountaine (Clark) and Robert Fountaine; her brother Robert Fountaine (Dea); and her niece Dreya Imani Fountaine.

Most recently, Carla held board roles for All People Arts, the Classic for Columbus, Parsons Avenue Merchants Association, Parsons Avenue Redevelopment Corporation, Rickenbacker Woods Foundation and South End Café.

“I was very sad to hear this news. I met Carla 20 years ago. She was fresh out of college beginning her career here at Nationwide Children's Hospital (NCH) and I was just beginning what I call my second career here at NCH. We met in orientation on Mon., April 26, 2004, and we had a fast friendship from there. Carla was smart and a go-getter. She was determined and persevered. She was definitely a sweetheart. With all she dealt with, she was the strongest person I know. Missing her. Love. Ann”
