He is survived by: his wife Noella Turcot; his children, Audie Wooten (Bethanne) and Rita Shapter; his grandchildren, Jonas Wooten, Jacob Wooten, Gabe Wooten, Abby Wooten (Anthony Williams), Breanna Meekings (Jai) and Miranda Shapter; his great grandchildren, Tess (Creamer), Kendrick, Mina Williams, Emerson and Carson Meekings; and his sister-in-law Mona (Tom).
In lieu of flowers, donations may be made to Jack's name to Nationwide Children's Hospital.
