He was predeceased by: his parents, Norman Lewis and Irene Lewis (Hummel); his brother Thomas Lewis; and his granddaughter Avery Lewis. He is survived by: his wife Daisy Lewis of Fostoria; his children, Dawn Hartley Jr. (Jack) of Fostoria and Jason Lewis of Tiffin; his grandchildren, Carmen and Victoria Lewis; and his brother Larry Lewis (Jean) of Bowling Green. He is also survived by nieces and nephews.
