He was predeceased by: his parents, Martin Louis and Blanch Elvelma Cook, Sr. (Seman); his son Rodney William Cook; and his siblings, Dorothy Jean Gildow, Martin Louis Cook and Helen Louise Byrly.
He is survived by: his children, Sheri Lyn Cassidy of Waynesville, NC, Linda Sue Schurch of West Chester, OH, Connie Jean Cook of Menlo Park, CA, Robyn Elaine Cook of Weaverville, NC and John Mark Cook; and his close friend Ann Thomas. He is also survived by eight grandchildren, one great grandson.
