He is survived by: his wife Patti Gentry Blake; his children, Mary Jo Keppner, Melinda Blake, Diana Holder (William) and Richard Blake (Tabitha); his grandchildren, Courtney, Katelyn, Richard, Seaira, Harlie, Emma and Lily; his great grandchildren, Tanner, Ila, Case, Vincent and River; and his siblings, Buddy Polite (Mary Ellen), Doris Souder and Patti Jones. He is also survived by Numerous Nieces, Nephews, other family and friends.
