He is survived by: his daughter Susie; his wife Grace Sly (Eckert); his daughter Susan Sly Burke; his daughter-in-law Linda Sly; his grandchildren, Rachel Putnam (Ralph Rupp), Anjanette DuSold, George DuSold, Ben Sly (Jamie), Trevor Sly (Kayla), Carli Decker (Eli), Phillip Gamboe, Ward Gamboe (Aundrea) and Logan Gamboe (Alyssa); his great grandchild Zachery Putnam; and his grandchildren, Braylon, Brogan, Bexsly Sly, Brianna Feathers, Landyn, Lyneah Sly, Daniel, Charlotte Grace Decker, Cailynn, Ariya, Demi Jo Gamboe, Benjamin, Alaina, Ramsey Gamboe and Koen Gamboe.
