He is survived by : his wife Barbara Czachorski Ampulski; his daughters, Sheri Terbovich (Jim), Staci Jarvis (Sean) and Gina Nilsson (Kevin); his grandchildren, Morgan, Danielle, Rachel, Alec Terbovich, Zack, Tori, Kennedy Jarvis, Aidan and Corey Nilsson; his siblings, Gary, Jim, Tim Ampulski and Janice Wilson (Steve); his brothers-in-law, Thomas Jarvis and Robert Czachorski (Connie); and his caregivers, Cecilia and Marcia.

Visitation was held on Tuesday, July 2nd 2024 from 6:00 PM to 8:00 PM at the Zettler Funeral Home (2646 Pleasant Ave, Hamilton, OH 45015). A prayer was held on Wednesday, July 3rd 2024 at 9:30 AM at the same location. A Mass of Christian burial was held on Wednesday, July 3rd 2024 at 10:00 AM at the Sacred Heart Church (400 Nilles Rd, Fairfield, OH 45014).
