He was predeceased by: his parents, Homer Paul and Beatrice Marie Pack (Conley); and his brothers, Sam, Brian and Bart.
He is survived by: his wife Carol; his children, Jasmine Pack, Matthew Spurlock (Carolyn) and Kelly McGough (Greg); his grandchildren, Tyler, Alayna, Emery Spurlock and Sage Gomez; his siblings, Nina Keegan (Mike), Bruce, Barry (Bonnie) and Ritchie (Debbie); and his siblings-in-law, Amy Barr, Roberta Lewis and Tim Stanislo. He is also survived by numerous nieces, nephews, other extended family, and many friends.
In lieu of flowers, memorial contributions may be made to Hospice of Dayton.
