He is survived by: his parent Paul Crooks; his son Nick Crooks of Beavercreek; his mother Pat May (Chuck) of Fairborn; and his sisters, Teri Kennard of Dayton and Cindy Stout (Steve) of Springfield. He is also survived by nieces, nephews, other extended family, and friends.
