He was predeceased by: his father Roland J. Herbert. He is survived by: his children, Kimberly Herbert and Hailey Herbert; his mother Martha L. Herbert; his siblings, Ron Herbert (Helen), Terilyn Egan (Tod) and Christina Herbert; his nieces and nephews, Shannon, Kevin, Rachel, Andrew, Katlyn, Anthony, Ashley, Justin, Daniel, Gregory and Brandon; and his great-nieces and great-nephews, Mickayla, Brettley, Jaxon, Coby and Greyson.
