He was predeceased by: his grandparents, Andrew A. Yakubik, Mary Ann Liptak and John A. Liptak; and his cousin Danielle Eibel Maximo. He is survived by: his parents, Aaron D. Yakubik and Teresa J. Yakubik (Liptak) of Byesville, OH.; his brother Jerod M. Yakubik of Byesville, OH.; and his grandmother Helen M. Yakubik of Cambridge, OH.. He is also survived by several aunts, uncles and cousins.
