He is survived by: his parents, Jerry Bookout and Patsy Bookout (Hensley); his wife Shannon Bookout; his son Zachary Bookout; his sisters, Lisa Artrip (Dwight), Leslie Thornton (Roger) and Patricia Randall (Chris); his daughters, Brittnee, Jessica and Kaitlyn; his mother-in-law Pamela Ash; and his siblings-in-law, Jason Ash and Sue Holland (Pete). He is also survived by many aunts, uncles, nieces, nephews and cousins.
