He was predeceased by: his mother Martha C. Kozen. He is survived by: his father Martin S. Kozen; his sister-in-law Phillip Kozen (Jennifer); his uncles, Sonya "Sonnie" Kasch (William) and Norman Kozen; and also, Samantha Kozen and Benjamin Kozen. He is also survived by other relatives and friends.

In lieu of flowers, memorial contributions may be made to Dayton Children's Hospital.
