He was predeceased by: his parents, Carlos Velazquez and Herminia Velazquez. He is survived by: his children, Michael Velazquez (Erika), Victoria Rivera, Jacqueline Payne and Dora Maysonet; his grandchildren, Megan, Eric, Michael, Halle, Ericka, Morgan, Erin, Nicole and Monika; his great grandchildren, Jaidyn and Ava; his brothers, Carlos Velazquez (Carmen) and Rafael Velazquez (Glenda); and his significant other Linda Gartman. He is also survived by many nieces and nephews.
