He was predeceased by: his parents, John Captain and Margaret Captain; and his sister Carol Furst. He is survived by: his sisters, Nancy Captain of Elyria, Jane McCarthy of Nashville, TN and Allie Furst of Cincinnati. He is also survived by 3 nieces and 2 nephews.
