He is survived by: his son Ernest Forman Jr. of Crystal Lake, Illinois; his grandchildren, Dylan Forman of Illinois and Jessica Flotterod (Brian) of Minnesota; his great grandchildren, Emma, Everet and Ethan; and his brothers, Jennings Forman of Florida and Walter Forman (Mae) of Florida. He is also survived by many nieces and nephews.
