He is survived by: his mother Antonina Kotula (Tluczek); his siblings, Richard Kotula, John Kotula (Karen), Marysia Golos (Peter) and Mark Kotula (Therese); his wife Terri Kotula (Grundish); his children, Jade Bentley (Tom) and Aaron Kotula (Chloe); and his granddaughters, Courtney, Lara and Asha.
