He was predeceased by: his wife Mary; and his son Michael. He is survived by: his daughters, Pamela Melvin and Jennifer Goehring; his children-in-law, Rick Melvin and LeeAnn Pfeiffer; and his grandchildren, Michael, David Melvin, Matt, Josh Pfeiffer and Ashley Van Goehring. He is also survived by his seven great grandchildren.
In lieu of flowers, the family requests donations be made in Phil's name to the Alzheimer's Association.
